Discovery Flight Liability Waiver

1. Acknowledgment of Risk

I, the undersigned, acknowledge that flight training and aircraft operations involve inherent risks, including but not limited to those caused by terrain, weather, equipment failure, and human error. I voluntarily assume all risks associated with my participation in this discovery flight.

2. Release of Liability

I hereby release, waive, and discharge Skyhawk Aviation, its owners, flight instructors, employees, agents, and representatives from any and all liability, claims, or causes of action for personal injury, death, or property damage arising out of or related to my participation in flight activities, to the fullest extent permitted by applicable law.

3. Medical Fitness

I certify that I am in good physical condition and do not have any medical condition that would interfere with my safety or the safety of others during this flight. I agree to notify the flight instructor of any condition that may affect my ability to safely participate in the flight.

4. Signature & Guardianship

By signing below, I acknowledge that I have read and understand the terms of this waiver and voluntarily agree to them. If the participant is under the age of 18, a parent or legal guardian must sign on the participant's behalf and accepts responsibility and liability for the minor to the extent permitted by law.
